I certify that the requested account(s) will be used for the work specified, and that the proposed CSU computing serviced is justified by the California State University System.

Unauthorized use of the CSU computing facilities is a violation of Penal code 502 and therefore punishable by a fine not exceeding $5,000.00 and/or imprisonment not exceeding three years and cancellation of accounts.

I understand that this form will result in the creation of a Central UNIX account, an Oracle Email Account, and an Oracle Calendar Account.
