Our Telephone Web forms have been removed as web forms, and have been replaced by our new Portlet.
To enter requests for any/all Telephone related requests, please subscribe to the Telephone Requests portlet on the Cal Poly portal.
NOTE: Only Telephone Coordinators will have access to the full portlet.
- Log into the Cal Poly portal.
- Select Customize in the upper right hand corner.
- A drop down portlet menu will appear.
- Select the Add Portlet tab on the left.
- Search Telephone Requests and click "Add."
The Telephone Requests portlet should now be available on your Cal Poly portal.
Need to report a problem with your campus phone line?
If you are not the Dept. Telephone Coordinator and do not have complete access to the portlet, you can still report a problem for your phone line. Use the instructions above to subscribe to the portlet, then select Desk Phone Service and select Report Service Trouble.
