Telephone Forms

Our Telephone Web forms have been removed as web forms, and have been replaced by our new Portlet.

To enter requests for any/all Telephone related requests, please subscribe to the Telephone Requests portlet on the Cal Poly portal. 

NOTE: Only Telephone Coordinators will have access to the full portlet.

To Subscribe:

  1. Log into the Cal Poly portal.
  2. Select Customize in the upper right hand corner.

  3. A drop down portlet menu will appear. 
  4. Select the Add Portlet tab on the left.

  5. Search Telephone Requests and click "Add."

The Telephone Requests portlet should now be available on your Cal Poly portal.

Need to report a problem with your campus phone line?
If you are not the Dept. Telephone Coordinator and do not have complete access to the portlet, you can still report a problem for your phone line. Use the instructions above to subscribe to the portlet, then select Desk Phone Service and select Report Service Trouble
