This form must be submitted by the Campus IT Coordinator (CITC). Please allow three business days for processing.

It is the department’s choice and responsibility to determine the model selection, data plan, accessories, peripherals (includes assistive technologies), warranty, applications, funding and technical support.

All models include 1-Year Apple Hardware Warranty and 90-Day Apple Telephone Support.
Sales tax is NOT included.
